Throughout 2025, Cornerstone remained active, engaged, and deeply connected to the individuals and families we serve. This newsletter shares what life at Cornerstone looks like — not as a report, but as a story of people, progress, and community.

🧩 Our Core Services: Care That Meets People Where They Are

Cornerstone Community Care offers a continuum of outpatient services designed to support adults at different stages of recovery and wellness. Our programs are person-centered, flexible, and grounded in real-life needs.

🏠 Structured Activities of Care – Outpatient (SACOT)

Substance Abuse Comprehensive Outpatient Treatment (SACOT) program means a periodic service that is a time-limited, multi-faceted approach treatment service for adults who require structure and support to achieve and sustain recovery.   

🌱 Substance Abuse Intensive Outpatient Program (SAIOP)

Substance Abuse Intensive Outpatient Program (SAIOP) means structured individual and group addiction activities and services that are provided at an outpatient program designed to assist adult and adolescent beneficiaries to begin recovery and learn skills for recovery maintenance.

💬 Outpatient Therapy & Counseling

Outpatient therapy provides individualized counseling for adults seeking support for substance use, mental health, or co-occurring challenges. Services focus on insight, emotional regulation, and practical strategies for daily life.

Support looks different for everyone — our services meet people where they are.

🌍 Accessibility, Resources & Continuity of Care

Cornerstone Community Care is committed to ensuring services are accessible and inclusive. Bilingual staff are available to assist clients with assessments, counseling, education, and referrals, helping reduce language barriers and improve understanding.

Support also extends beyond the agency. Our team actively networks with Anson County food banks, local crisis resources, and community supports in surrounding areas. Clients receive assistance connecting to Non-Emergency Medical Transportation (NEMT), transitional housing, and affordable housing resources, Anson county food bank, helping reduce barriers and support continuity of care..

  • January marked a fresh start and new beginnings across the Cornerstone community. Individuals engaged in supportive outpatient services designed to meet people where they are, including Substance Abuse Comprehensive Outpatient Treatment (SACOT) and the Substance Abuse Intensive Outpatient Program (SAIOP). These services are paired with individual and group therapy, offering support, structure, and encouragement in a welcoming environment.

    Continuity of care remained a priority, with monthly visits from a physician, the local health department, and dental professionals. Individuals were also supported through connections to local food banks and Non-Emergency Medical Transportation (NEMT) services to help reduce barriers and promote consistent access to care. These supports reinforced health education, medication education, and dental awareness, helping individuals stay informed and supported in their overall wellness.

    January set a positive tone for the year, focused on stability, connection, and growth.

  • February built on that foundation with growing engagement and consistency. Individuals continued participating in SACOT, SAIOP, and therapy services, while also benefiting from coordinated wellness supports that reinforce whole-person care.

     

    Health, medication, and dental education continued through monthly visits, along with ongoing support connecting individuals to food banks and transportation resources as needed. February also included shared activities and moments of celebration that encouraged peer connection, support, and community spirit.

    This month reflected growing confidence and momentum as individuals continued working toward wellness and recovery goals.

  • March brought increased energy and connection across the Cornerstone community. Engagement in SACOT, SAIOP, individual counseling, and group therapy continued, supported by consistent access to health, medication, and dental education through visiting professionals.

    Support with food resources and Non-Emergency Medical Transportation (NEMT) continued to strengthen continuity of care. As a month focused on awareness and growth, March emphasized the importance of emotional wellness, self-care, and continued connection. Birthdays and milestones were celebrated, reinforcing encouragement and a sense of belonging.

    March also reflected Cornerstone’s continued commitment to providing coordinated care in a supportive, community-centered environment.

    Cornerstone Community Care is proud to support individuals through a coordinated approach that brings services and wellness supports together. In addition to outpatient services and therapy, individuals benefit from regular access to medical, health, and dental education, as well as assistance connecting to local food banks and Non-Emergency Medical Transportation (NEMT) services — helping bridge care, reduce barriers, and promote overall wellness.

    This integrated approach supports stability, informed self-care, and long-term success.

  • April emphasized balance, reflection, and emotional wellness. Individuals continued engaging in Substance Abuse Comprehensive Outpatient Treatment (SACOT) and the Substance Abuse Intensive Outpatient Program (SAIOP), alongside individual and group therapy, in a supportive and encouraging environment.

    Monthly visits from a physician, the local health department, and dental professionals supported ongoing health education, medication education, and dental awareness. Assistance with food resources and Non-Emergency Medical Transportation (NEMT) services supported continuity of care and access to services.

  • May highlighted Mental Health Awareness Month, bringing added focus to emotional wellness, coping skills, stress management, and self-advocacy. Participation in SACOT, SAIOP, and therapy services continued, supported by coordinated wellness education, food resource connections, and transportation support.

    A Memorial Day Cookout created an opportunity for reflection, connection, and togetherness, reinforcing the importance of community and shared support.

    Memorial Day is a time to honor and remember those who gave their lives in service to our country. At Cornerstone, this day also serves as a reminder of the importance of reflection, gratitude, and community. Coming together during this time reinforced shared respect, appreciation, and the value of supporting one another through connection and understanding.

  • June was a month of celebration and joy. A baby shower and birthday recognitions brought people together to celebrate milestones and new beginnings. Engagement in SACOT, SAIOP, individual counseling, and group therapy continued, supported by consistent access to health, medication, and dental education, along with ongoing assistance connecting individuals to food banks and NEMT services.

    June reflected growth, encouragement, and continued connection across the Cornerstone community.

  • July brought community pride and celebration. A July 4th Cookout created space for connection, shared enjoyment, and togetherness. Individuals continued engaging in Substance Abuse Comprehensive Outpatient Treatment (SACOT), SAIOP, individual counseling, and group therapy, supported by ongoing wellness education, food resource connections, and transportation assistance.

    The Fourth of July represents freedom, resilience, and the strength of community. Celebrating together provided an opportunity to recognize independence, personal growth, and the collective progress made throughout the year. The gathering reflected unity, encouragement, and the importance of shared moments that bring people together.

  • August focused on maintaining consistency and momentum. Participation in SACOT, SAIOP, and therapy services continued alongside coordinated wellness supports, including assistance with food banks and Non-Emergency Medical Transportation (NEMT) as needed. Community presence and connection remained strong, reinforcing encouragement and stability.

  • September reflected continued progress and meaningful engagement. Individuals remained connected to services and wellness supports, including food and transportation resources, highlighting Cornerstone’s focus on continuity of care.

    This month also marked an important organizational milestone as Cornerstone submitted its CARF accreditation application, reflecting growth and ongoing commitment to quality and accountability.

  • October continued conversations around mental health awareness and emotional wellness. Engagement in Substance Abuse Comprehensive Outpatient Treatment (SACOT), SAIOP, individual counseling, and group therapy continued, supported by monthly visits from a physician, the local health department, and dental professionals.

    Support with food resources and Non-Emergency Medical Transportation (NEMT) continued, strengthening continuity of care. Cornerstone also participated in the local Town Festival, reinforcing community presence and connection.

  • November centered on gratitude, inclusion, and reflection. A Thanksgiving Dinner brought people together to share a meal and celebrate progress. Participation in the North Carolina Hmong New Year Celebration strengthened cultural connection and community partnerships.

    Ongoing assistance with food banks, transportation resources, and wellness supports continued to promote stability and continuity of care.

    Thanksgiving is a time to pause and reflect on gratitude, connection, and shared support. It offers an opportunity to recognize progress, appreciate one another, and acknowledge the relationships that strengthen our community. Coming together during this season reinforces the importance of compassion, inclusion, and giving thanks for the journey we share.

  • December was a time of reflection, hope, and celebration. A Christmas Party highlighted connection, appreciation, and shared accomplishments. Engagement in SACOT, SAIOP, and therapy services continued alongside health, medication, and dental education, with continued support connecting individuals to food banks and Non-Emergency Medical Transportation (NEMT).

    December also included official notification that Cornerstone’s CARF on-site survey is scheduled for February 2, 2026, marking an important step forward for the organization.

    The Christmas season represents hope, generosity, and togetherness. It is a time to reflect on growth, celebrate milestones, and look ahead with optimism. This season reminds us of the importance of kindness, encouragement, and supporting one another, especially during moments of transition and renewal.

    Most importantly, December was a time to express sincere appreciation to every individual served, every family member, every partner, and every stakeholder who helped make this year possible. Together, we continue building a community of growth, support, and recovery.

As Cornerstone prepares for its CARF accreditation survey in February 2026, our focus remains on learning, growth, and providing high-quality, person-centered care. This journey reflects commitment — to clients, to community, and to continuous improvement.
